Code of Conduct / Billing Contract

We want to take a moment to remind you of the guidelines we have put in place to ensure a safe and respectful learning environment for all students and parents.

• All students and parents must always behave politely and respectfully.
• Parents are asked to remain outside after student drop off unless otherwise requested by an instructor.
• Students must remain in class until dismissed by the instructor.
• Proper footwear is required for all classes to participate.
• If your child wishes to participate in the recital, they must attend at least 50% of all classes.
• Students who miss the last four classes leading up to the recital will be placed at the back of all dances.
• If you miss the dress rehearsal, your child will not be able to participate in the recital, regardless of costumes being purchased.
• We reserve the right to dismiss any parent or child who does not follow these guidelines.
• Deep Studios is not responsible for any accidents, you must dance at your own risk.
• I understand that payments are non-refundable and if my child attends class within the month I am responsible for the whole month regardless of attendance and I am responsible to find a recital sponsor or pay a $30 recital fee per dancer.

I hereby release Deep Studios, employees from all liability for personal injury, illness or property damage occurring on or off the studio's premises. I have read Deep Studios General information and policies as outlined. I certify that my student is in good health and capable of participating in physical activities. I hereby give permission to Deep Studios to take and use photographs for promotional use of the studio.
